Supporting Health Through Physical Activity

Live Well works alongside health, social care and community professionals to support individuals living with long-term health conditions, disabilities and those who would benefit from increased physical activity and social connection.

Our programmes provide safe, supportive and accessible opportunities that help people improve their physical health, mental wellbeing and confidence.

Activity Referral Programme

The Activity Referral Programme provides supported access to physical activity opportunities for people living with health conditions or disabilities.

Participants can access:

  • Specialist health classes
  • Supported gym programmes
  • Swimming opportunities
  • Individual guidance from Health Instructors

The programme helps people build confidence, improve wellbeing and become more active in a safe and supportive environment.

Social Prescribing

Live Borders supports local social prescribing pathways by helping people access physical activity, wellbeing opportunities and community support.

Working alongside healthcare professionals, Community Link Workers and partner organisations, we help individuals connect with activities and opportunities that can improve health, wellbeing and quality of life.

This support complements wider health and social care services and can help reduce isolation, improve confidence and encourage healthier lifestyles.

Eligibility

Individuals aged 16 and over living with a health condition, disability or circumstance where increased physical activity would support health and wellbeing.
